The Alzheimer’s Association has been awarded a 2 year Dementia Care Practice Recommendations (DCPR) grant. We will invite 25 Washington nursing homes to be engaged in a strength-based coaching model to increase or maintain person-centered care practices as outlined in the DCPR. Over a six month coaching period, nursing homes will work with the Alzheimer’s Association coaches to assess current level of adoption of DCPR and prioritize areas of need/focus. Through an action planning and implementation process, nursing home staff will make organization-wide measurable changes. 

 

This opportunity is based in the Lynnwood, WA office with a significant amount of travel within the state of Washington to the 25 care centers with overnight travel required. 

 

This role will be hired at a Grade 6 with a starting point of $52,500, and a mid salary range of $66,000 plus our great benefit package. 

 

 

 

ESSENTIAL FUNCTIONS: 

 

-Participate in all relevant trainings focused on coaching skills and understanding DCPR.  

 

-Recruit 25 Skilled Nursing Facilities (SNF) across Washington State. 

 

-Establish effective communications with professional caregiver teams to provide coaching on DCPR, via monthly in-person and/or video conferencing meetings and telephone and email support.  

 

-Abe to learn, teach others, and effectively utilize technology and databased tracking systems. 

 

-Participate in research-driven discussions and put recommended DCPR tools and strategies into place, and providing evaluation, assessment and guidance on grant guidelines and methods. 

 

 -Understands and able to describe and adhere to cultural change and person-centered care practices. 

 

-Able to work independently and collaboratively on a team and with multiple leaders and take direction from the coach coordinator at the home office. 

 

-Identify and meet the needs of diverse communities and populations within the context of the  

 

-skilled nursing home setting. 

 

-Represent the Alzheimer’ Association chapter at appropriate and assigned in-person and virtual meetings and events in a professional manner. 

 

 

 

MINIMUM REQUIREMENTS 

 

-A Master's Degree in social services or a related field preferred or equalvent experience. Experience working with or for a residential setting a plus. 

 

-A minimum of two to three years of experience in community outreach activities, and competency in teaching, training and/or delivering educational programs or public speaking.  

 

-Ability to work in recording and monitoring systems as needed per grant requirements. 

 

-Ability to lead, guide and inspire nursing home staff to achieve goals and objectives while demonstrating and promoting open communication, transparency and teamwork towards the goals of the grant. 

 

-Ability to multi-task, be flexible, and handle changing priorities in a fast-paced environment. 

 

-Excellent written, verbal and interpersonal communication skills. 

 

-Access to a reliable vehicle, a valid driver’s license and proof of automobile insurance 

 

-Ability for overnight travel, evenings and weekends as needed.
